Job summary

Brooksby Village by Erickson Senior Living in Peabody, MA, is seeking a Memory Care Manager to lead a 44-bed Memory Care neighborhood and champion person-centered care for residents living with dementia.

You will oversee quality, performance, and resident experience while integrating Memory Care best practices across Skilled Nursing and Assisted Living neighborhoods, collaborating with Wellness Managers, families, and staff to meet psychosocial, clinical, and emotional needs with compassion.

Qualifications

• Minimum of 3 years related experience in long term care/assisted living.
• Minimum of 3 years related experience in providing services for residents with cognitive impairment.
• Current CPR certification preferred.
• Knowledge of the geriatric population and the aging process, including physical, psychological, and social needs.
• Demonstration of progressive approaches to supporting and enhancing quality of life for seniors with cognitive impairment.

Responsibilities

• Conduct pre-residency assessments and screening of potential residents and make recommendations for appropriate Admissions.
• Evaluate psychosocial and ADL needs of Memory Care residents and coordinate care and services.
• Monitor engagement of activities for residents with cognitive impairment and participate in program development and planning.
• Lead a holistic, person-centered approach to assessment and care planning.
• Partner with departments to develop support groups and educational programming for residents and families in Memory Care.
• Support transitions of residents between levels of care with Social Workers.

As the leader of the Memory Care neighborhood, you’ll oversee its overall management, performance, quality, and resident experience while also supporting the integration of Memory Care best practices throughout our Skilled Nursing and Assisted Living neighborhoods. You’ll partner closely with Wellness Manager, residents, families, and team members to ensure their psychosocial, clinical, and emotional needs are met with compassion and dignity.

How you will make an impact

• Conducting pre-residency assessments and screening of potential residents and making recommendations for appropriate Admissions
• Evaluating the psychosocial and activity of daily living (ADL) needs of residents in Memory Care and coordinating care and services
• Monitoring the engagement of activities for residents with cognitive impairment, as well as participating in program development and strategic planning
• Leading a holistic, Person-Centered approach to assessment and care planning
• Partner with other departments to develop support groups and educational programming opportunities for residents and family members in the Memory Care neighborhood.
• Supporting Independent Living and Post-Acute Social Workers to effectively transition residents between levels of care
• Maintaining a working knowledge and ensures compliance of Federal, State, and local regulations, as well as facility policies regarding Memory Care (and level of care – LTC or ALF)

Compensation

Salary range $90-105k/year with eligibility for annual bonus.

What you will need
