Job summary

Living Branches in Souderton, PA is hiring a full-time LPN/RN for our New Memory Care Unit. You will work on 1st and 2nd shifts, building relationships with residents and delivering person-centered care.

You will utilize the nursing process, monitor vital signs, respond to emergencies, and collaborate with the team to develop and evaluate care plans. An active LPN/RN license and 2–4 years of related experience are preferred. Join a 5-star CMS-rated community and grow with us.

Qualifications

• Active LPN or RN license in good standing.
• 2–4 years of related experience preferred.
• Ability to manage multiple priorities and respond to emergencies.
• Knowledge of HIPAA and long-term care regulations.
• Strong communication and teamwork skills.

Responsibilities

• Utilize nursing process to evaluate resident needs and develop care plans.
• Administer nursing care and evaluate outcomes.
• Monitor vital signs and complete readings, including pacemaker checks.
• Respond to emergency calls and notify physicians or supervisors as needed.
• Maintain HIPAA compliance and care regulations.

LPN/RN Overview:

• Utilize the nursing process, procedures and techniques to perform various duties such as evaluating resident needs, developing individual care plans, administering nursing care, and evaluating nursing care


• Ensures that temperature, pulse, blood pressure and other vital sign checks including pacemakers checks are completed and recorded


• Responds and reacts to emergency calls from residents.


• Notifies physicians, supervisor, family or other appropriate personnel when unusual resident condition exists.


• Valid LPN/RN License


• 2-4 years of related experience (Preferred)


• Ability to handle multiple responsibilities simultaneously, has ability to prioritize work and address emergencies as they may occur


• Knowledge of HIPPA rules and regulations and of regulations and guidelines governing personal care and/or long-term care
