Job summary

National Church Residences is seeking a Part-Time Licensed Practical Nurse for Memory Care, 7p–11p, in Columbus, Ohio. You will provide nursing care under the plan of care, document timely, and support residents and families.

The role includes teaching and mentoring direct care staff, coordinating with RN Case Manager, participating in team meetings, and ensuring compliance with policies and HIPAA; requires active LPN license and 12 months of nursing experience.

Qualifications

• Licensed Practical Nurse with current LPN license.
• 12 months facility/agency nursing experience preferred.
• Must have good verbal and written communication, comprehension, computer and interpersonal skills.

Responsibilities

• Provides direct patient care per the plan of care and physician orders.
• Implements plan of care initiated by the RN.
• Maintains accurate and timely documentation.
• Educates residents and families about diagnosis and plan of care.
• Coordinates home health services and communicates needs to RN Case Manager.
