Job summary

New Mark Rehab and Healthcare Center is looking for a Memory Care LPN to join our Missouri team. The role focuses on dementia and memory-related resident care with patient, compassionate service and adherence to individualized care plans.

We offer 12-hour shifts in a full-time, part-time or PRN capacity, with competitive pay and a comprehensive benefits package. You will collaborate with RNs, physicians, CNAs, and families to support residents' physical and emotional well-being.

Qualifications

• Current LPN license in Missouri is required.
• Experience in memory care, dementia care, long-term care, rehab, or similar setting preferred.
• Strong communication, clinical, organizational, and leadership skills.
• Compassionate, patient-centered approach to resident care.
• Ability to work effectively as part of a team.
• Commitment to safe, respectful, and individualized care.

Responsibilities

• Deliver high-quality, compassionate nursing care per individualized care plans.
• Provide person-centered care to residents with dementia and memory-related conditions.
• Assist residents with ADLs while preserving dignity and independence.
• Promote resident engagement and meaningful activities for well-being.
• Create and maintain a safe, secure environment for residents.
• Follow nursing process to assess, plan, implement, and evaluate care.
• Monitor changes in residents and respond appropriately.
• Document resident history, vital signs, and observations.
• Administer medications and treatments as ordered.
• Communicate with RNs, physicians, CNAs, families, and team members.
• Guide and support nursing staff to ensure consistent care.

Responsibilities

• Deliver high-quality, compassionate nursing care to residents in accordance with individualized care plans
• Provide person-centered care to residents living with dementia and other memory-related conditions
• Support residents with activities of daily living (ADLs) while promoting dignity, independence, and respect
• Promote resident engagement, socialization, and meaningful activities to support mental and emotional well-being
• Create and maintain a safe, secure, and supportive environment for residents
• Follow the nursing process to assess, plan, implement, and evaluate individualized care
• Monitor residents for changes in condition, behavior, mood, or overall well-being and respond appropriately
• Accurately document resident history, vital signs, clinical observations, and changes in condition
• Administer medications and treatments as ordered and in accordance with applicable policies and regulations
• Communicate effectively with RNs, physicians, CNAs, CMTs, families, and interdisciplinary team members
• Provide guidance and support to the nursing team to ensure consistent, high-quality resident care

Qualifications

• Current LPN license in the state of Missouri (Required)
• Previous experience in memory care, dementia care, long-term care, assisted living, rehab, or a similar healthcare setting preferred
• Strong communication, clinical, organizational, and leadership skills
• Compassionate and patient-centered approach to resident care
• Ability to work effectively as part of a team
• Commitment to providing safe, respectful, and individualized care
