Job summary

New-Mark-Rehab-and-Healthcare-Center in Missouri is seeking an experienced Memory Care LPN to provide compassionate, person-centered nursing care to residents with dementia.

You will work 12-hour shifts in a supportive team, document care, administer medications, and collaborate with RNs, physicians, and CNAs to promote safety and quality of life.

Qualifications

• Current LPN license in Missouri (Required).
• Previous memory care, dementia care, long-term care, assisted living, rehab, or similar healthcare setting preferred.
• Strong communication, clinical, organizational, and leadership skills.
• Compassionate and patient-centered approach to resident care.
• Ability to work effectively as part of a team.
• Commitment to providing safe, respectful, and individualized care.

Responsibilities

• Deliver high-quality, compassionate nursing care to residents in accordance with individualized care plans.
• Provide person-centered care to residents living with dementia and other memory-related conditions.
• Support residents with ADLs while promoting dignity, independence, and respect.
• Promote resident engagement, socialization, and meaningful activities to support mental and emotional well-being.
• Create and maintain a safe, secure, and supportive environment for residents.
• Follow the nursing process to assess, plan, implement, and evaluate individualized care.
• Monitor residents for changes in condition, behavior, mood, or overall well-being and respond appropriately.
• Accurately document resident history, vital signs, clinical observations, and changes in condition.
• Administer medications and treatments as ordered and in accordance with applicable policies and regulations.
• Communicate effectively with RNs, physicians, CNAs, CMTs, families, and interdisciplinary team members.
• Provide guidance and support to the nursing team to ensure consistent, high-quality resident care.
