Trilogy Health Services, LLC seeks a Memory Care Leader for Wellbrooke of Wabash, IN to oversee the Legacy Lane dementia care program. You will lead staff, develop personalized plans, and support families through the care continuum.

The role requires coordinating with leadership, ensuring dementia best practices, and maintaining a safe, compassionate memory care environment with ongoing staff education.

Qualifikationen

• High School diploma or GED preferred.
• CNA, QMA, LPN or RN license may be held in the state practiced.
• 1–3 years of relevant experience preferred.

Aufgaben

• Oversee daily operations of the Legacy Lane memory care program.
• Educate campus staff on dementia care and act as Dementia Champion.
• Maintain resident population per memory care move-in/out criteria.
• Develop personalized Service Plans and guide families through care continuum.
• Ensure dementia-safe environment with daily rounds and safety monitoring.
