Job summary

Heritage Operations Group LLC in Decatur, IL seeks a Memory Care Director to lead our dementia-focused memory care community. You’ll combine your nursing expertise with servant leadership to deliver compassionate resident care, engaging programming, and a warm, dignified environment.

You’ll supervise CNAs and nursing staff, conduct assessments and care plan meetings, partner with families and physicians, and drive dementia-care best practices across the team.

Qualifications

• LPN license and nursing degree required.
• Minimum 2 years of nursing experience.
• Leadership or supervisory experience preferred.
• Experience in memory care, assisted living, or senior living preferred.
• Strong knowledge of dementia care and resident-centered services.
• Excellent communication, organizational and problem-solving skills.
• Proficiency with EHR documentation.

Are you a compassionate Licensed Practical Nurse (LPN) with a passion for dementia care and leadership? We're seeking a dedicated Memory Care Director to lead our specialized memory care unit, ensuring residents receive exceptional care, engaging programming, and a meaningful quality of life.

In this rewarding leadership role, you'll combine your clinical expertise with your ability to build relationships, mentor caregivers, support families, and create a warm, person-centered environment for residents living with Alzheimer's disease and other forms of dementia.

What You’ll Do

• Lead and oversee daily operations of the memory care community
• Supervise, coach, and support CNAs and nursing staff
• Conduct resident assessments, care plan reviews, and service plan meetings
• Partner with residents, families, physicians, and interdisciplinary team members to ensure quality care
• Develop and implement engaging, dementia-focused programming and activities
• Monitor resident wellbeing through daily rounds and ongoing evaluations
• Coordinate staffing, scheduling, training, and performance management
• Provide education and guidance to staff on dementia care best practices
• Foster a safe, welcoming, and dignified environment for residents

What We’re Looking For

• Graduate of an accredited nursing program
• Minimum of 2 years of nursing experience
• Leadership or supervisory experience preferred
• Experience in assisted living, memory care, senior living, or long-term care strongly preferred
• Strong knowledge of Alzheimer's disease, dementia care, and resident-centered services
• Excellent communication, organizational, and problem-solving skills
• Ability to build positive relationships with residents, families, and team members
• Computer proficiency and documentation experience within an EHR

Pay Range The estimated pay range for this job. Disclosing pay information promotes competitive and equitable pay. The actual pay rate will depend on the person's qualifications and experience. $76,000.00 - $83,000.00 / year
