Job summary

Genesis is seeking a Memory Care Director (Dementia Program Director) for our Santa Fe, NM location. The role requires an LPN or RN and a minimum of 2 years in long-term care plus 1 year in memory care.

Qualifications

  • Two years in long-term care and one year in memory care.
  • Bachelor's or Master's in nursing, social work, gerontology, psychology, or related field preferred.
  • Three years of direct dementia care experience or approval by administrator.
  • Two years supervisory experience in a healthcare setting.

Responsibilities

  • Administers a Dementia Program addressing physical, social, cultural, spiritual, and intellectual needs.
  • Participates in staff meetings to coordinate quality patient/resident care.
  • Develops programming to maximize abilities, contentment, and self-esteem.
  • Performs quality improvement evaluations covering environment, documentation, team functioning, and care quality.
  • Develops daily and monthly activity calendars for the unit and participates in implementation.
  • Advocates for patients/families and provides education and support.
  • Manages admissions and discharges from the unit.
  • Oversees budget and financial performance for the Dementia Program unit.

Responsibilities

The Memory Care Director, titled Dementia Program Director within Company, administers a Dementia Program that therapeutically encompasses patients/residents' physical, social, cultural, spiritual, and intellectual needs.

  • Member of the of nursing center interdisciplinary team and as such participates in staff meetings to ensure coordination of quality patient/resident care.
  • Assess each patient/resident and develop programming that maximizes the customer's abilities and provides contentment, enjoyment, and positive self-esteem.
  • Perform ongoing quality improvement evaluations that include quality indicators for the environment, documentation, team functioning, and all aspects of quality care provision as well as reporting identified issues back to the team for resolution.
  • Develop daily and monthly activity calendar for the unit that includes both small and large group activities and participates in their implementation.
  • Advocate for patients/residents and their families and provides direct services to families through communication, education, and support.
  • Manage all patient/resident admissions and discharges from the unit.
  • Oversee the financial performance of the Dementia Program unit by developing and monitoring budget.
Qualifications
  • Must be a graduate of an accredited college or university with a Bachelor's or Master's degree in social work, nursing, therapeutic recreation, psychology, gerontology, or other related field preferred. *Three years of direct experience (in respective discipline) with dementia/patients who have dementia, and/or their families and caregivers, or approval by the Nursing Home Administrator/Executive Director. *Two years of supervisory experience in an appropriate health care setting, or approval by the Nursing Home Administrator/Executive Director.
  • Recommended state licensure in respective discipline is required.
  • Must meet state requirements for concurrent job responsibilities, if applicable.
