Job summary

PACS Group is seeking a Memory Care Director to plan, direct, and implement memory care programming across the community. The role requires supervising the memory care team and coordinating with health services to develop initial service plans and ongoing updates for residents.

The position involves collaborating with outside providers, ensuring safety systems, and maintaining regulatory compliance while supporting family communication and community engagement.

Qualifications

• Minimum requirements include a high school diploma; associate's or bachelor's degree preferred.
• Experience in memory care management with leadership abilities.
• Proven ability to communicate effectively with staff, residents and families.

Responsibilities

• Evaluate residents' health status and participate in individualized service plans.
• Hire, onboard, train, and schedule Memory Care team members.
• Create and lead monthly Memory Care programming using focus plans.
• Guide team in delivering personal care services with personalized plans.
• Support Health Services Director in initial service plans and updates.
• Coordinate with outside providers for resident care services.
• Provide on-call support for staff questions and staffing coordination.
• Complete incident and accident reports for Memory Care residents.
• Ensure resident safety systems are in place and operational.
• Collaborate to maintain regulatory compliance and surveys.
• Assist with memory care neighborhood labor and supply management.
• Communicate with families, staff, and providers; support community events.
