Job summary

Solera Senior Living''s Trulee Evanston in Evanston, IL is seeking a Director of Memory Care who will oversee the dementia care program, lead the Memory Care team, and collaborate with families and physicians to ensure exceptional resident outcomes.

The role includes assessing residents, developing care plans, ensuring regulatory compliance, and participating in growth initiatives while maintaining a compassionate, person-centered approach.

Qualifications

• College degree with dementia care coursework or 2+ years of management with persons with dementia (per Illinois regulations).
• Degree in social work, psychology, sociology, or gerontology preferred.
• Licensed nursing or healthcare professionals strongly preferred.

Responsibilities

• Plan, develop, and evaluate the Memory Care program (Aspyre neighborhood) in compliance with federal, state, and Solera standards.
• Conduct pre-move-in and ongoing resident assessments; develop and monitor individualized care plans.
• Monitor changes in resident condition and coordinate appropriate interventions.
• Maintain accurate documentation and complete all required data entry in resident care systems.
• Hire, train, schedule, supervise, and evaluate Memory Care team members.
• Provide orientation and ongoing dementia‑specific training in accordance with state and federal requirements.
• Ensure adequate staffing and coverage while adhering to budgets and policies.
• Coach team members to deliver compassionate, person‑centered care.
• Serve as primary resource for residents, families, and physicians.
• Lead service plan meetings and support ongoing family communication.
• Coordinate care services to ensure continuity and high standards of care.
• Support community inspections and ensure compliance with all regulatory and safety standards.
• Maintain a safe, engaging, and therapeutic environment tailored to residents with dementia.
• Participate in tours, marketing efforts, and referral development to support census growth.
• Promote teamwork, professionalism, and a positive community image.
• Perform other duties as assigned by the Executive Director
