Job summary

Touchstone Communities is seeking a compassionate Memory Care Director in Bulverde, TX. The role leads a dementia-focused memory care environment, ensuring safety, dignity and meaningful engagement for residents with dementia.

It requires clinical leadership, cross‑functional collaboration with families and the care team. You will design and implement stage-appropriate programs, mentor staff, and support families through the dementia journey with compassionate leadership and best-practice care

Qualifications

• Valid Texas LVN license required and compact licensure considered.
• Experience leading nursing teams in a skilled nursing facility.
• Proven ability to develop memory care programs and care plans.
• Must be available for clinical on-call rotation and emergencies.

As our Memory Care Director, you will be the guiding force behind a vibrant, engaging, and supportive Memory Care environment. Your leadership will help ensure every resident experience safety, dignity, and connection.

You will:

What You Bring:

• Valid Texas LVN license or compact nursing license — required
• Experience as a nurse leader in a skilled nursing facility and working directly with memory care patients required
• Must be available for clinical on-call rotation
• Strong leadership, communication, and team‑building skills
• Experience working with individuals living with dementia, including program development and understanding of disease progression
• A compassionate heart and a service‑driven mindset
• A commitment to ensuring every resident receives dignified, person‑centered care
