Live Well Homecare Agency, LLC in Phoenix, Arizona, is seeking an experienced leader to oversee resident care and daily operations in a memory care community. You will ensure high-quality support for residents with cognitive needs and coordinate care plans with the Wellness Director.

The role leads caregivers and medication technicians, maintains documentation, and addresses resident and family concerns with empathy and strong communication.
