Job summary

FORESTHILLS in Memory Care is seeking a dedicated Certified Nursing Assistant to provide routine daily nursing care to residents in accordance with assessment and care plans. You will identify residents, document care, and communicate changes to supervisors while maintaining confidentiality and a warm, supportive environment.

Responsibilities include assisting with daily activities, transporting residents, and helping prepare residents for medical tests, under supervision, following established

Qualifications

• Must possess a valid CNA license in accordance with state laws.
• Minimum 10th grade education.
• CPR certification preferred.

Responsibilities

• Identify residents before procedures using wristband/photo card.
• Record entries accurately on flow sheets, notes, or charts.
• Report changes in resident condition to Nurse Supervisor/Charge Nurse.
• Maintain confidentiality of resident information and report any disclosures.
• Provide daily routine nursing care as per care plans and policies.
• Assist with transferring and transporting residents between rooms and areas.
• Assist with daily activities and personal care as required.
• Prepare residents for medical tests and examinations as instructed.
