Job summary

Oakmont Management Group's Ivy Park at Cathedral Hill in California is seeking a Care Provider for Memory Care. This full-time role covers Tuesday–Saturday or Wednesday–Sunday shifts and involves assisting residents with daily living activities, healthcare coordination, and safety on a memory care unit.

The ideal candidate will have strong customer service, empathy for the elderly, and the ability to communicate with residents, families, and care teams.

Qualifications

• Experience preferred but not necessary; we will train the right candidate.
• Must be at least 18 years of age.
• High School Diploma or GED.
• Excellent customer service skills.
• Strong desire to work with the elderly and care for their needs.
• Possess written and verbal skills to interact and communicate effectively with employees, supervisors, physicians, healthcare professionals, residents, and their families.
• Must pass a Criminal Background check and Health Screening tests, including physical and TB Tests.
• Oakmont Management Group may require vaccination and PPE per regulatory guidelines.

Responsibilities

• Assist residents in Activities of Daily Living (ADL) per care plan (eating, bathing, dressing, toileting, transferring, continence).
• Promptly respond to residents' needs while maintaining dignity, safety, and confidentiality.
• Report observed changes in residents' condition to a supervisor.
• Maintain a safe, secure environment following safety standards.
• Encourage teamwork and cooperative interactions with staff and other departments.
