Job summary

Alden Poplar Creek Rehabilitation and Health Care Center is seeking a Memory Care Aide to support the Memory Care Unit’s person-centered program for residents with dementia. You will help implement therapeutic activities, communicate with families, and participate in multidisciplinary care planning.

Requirements include a high school diploma, healthcare experience, dementia exposure, and a driving license with willingness to drive the facility bus.

Qualifications

• Hold a high school diploma or equivalent.
• Have at least one year of experience in the healthcare field either working or volunteering.
• Have at least three months of experience engaging with persons with Alzheimer’s disease and other dementias.
• Must have the sincere desire to work with the memory care population.
• Be able to express the Memory Care Program to the community and professionals.
• Possess confidence to apply knowledge with residents and families.
• Willingness to continually learn about dementia and care approaches.
• Able to utilize therapeutic approaches for dementia care on an ongoing basis.
• Able to make independent decisions in residents' best interests.
• Demonstrate empathy, compassion, understanding, enthusiasm, and humor.
• Understand and manage emotional needs of yourself.
• Know how to deliver memory care programming on the unit.
• Have a valid driver’s license and willingness to drive the Alden bus.
• Read, write, and speak English.

Responsibilities

• Assist in providing a holistic, person-centered memory care program for residents with dementia.
• Support ongoing activities and evidence-based therapies on the memory care unit.
• Assist with activity assessments and develop care plans in collaboration with the team.
• Encourage family involvement through volunteering and visitation.
• Complete required documentation and maintain policy compliance.
• Provide emotional, caring support to residents, families, and staff.
• Be flexible to adapt to residents’ changing needs.
• Perform other duties as assigned.
