Job summary

Qualifications

• Associates Degree in Nursing or BSN is required.
• Current MA RN license and BLS required.
• Experience with elder population preferred and EMR experience preferred.

Responsibilities

• Perform initial and annual state clinical assessments (MDS-HC) for enrollees and participants.
• Complete forms and documentation to meet state review requirements.
• Maintain detailed submission records and state responses.
• Collaborate with departments to ensure enrollment transitions and data accuracy.
• Travel between Mattapan and Brockton sites and intake locations as needed.
