Job summary

Harbor Health’s Elder Service Plan in Mattapan and Brockton seeks a Membership/MDS Nurse to conduct state clinical assessments, ensure completion of forms, and support enrollment for PACE participants. The role requires travel between sites, strong documentation, and collaboration with interdisciplinary teams.

The Membership RN will navigate MDS-HC requirements, maintain EMR records, and ensure timely enrollment while supporting residents in the community.

Qualifications

• Associates Degree in Nursing or BSN required.
• Current MA RN license and BLS certification required.
• Experience with elder population preferred; EMR experience preferred.

Responsibilities

• Perform initial and annual state clinical assessments (MDS-HC).
• Complete and submit assessments for state review, and secure approvals.
• Travel between Elder Service Plan sites to enroll participants.
• Maintain detailed records of submissions and state responses.
• Provide clinical information to Interdisciplinary Teams for enrollment transitions.
