Qualifikationen
- Associates Degree in Nursing or BSN.
- MA RN license and BLS.
- Experience with elder populations preferred.
- Experience with MA Minimum Data Set – Home Care Clinical Assessment preferred.
- Proficiency with Office 365 including Word, Outlook, Excel and Teams.
Aufgaben
- Completes initial and annual state clinical assessments (MDS-HC) for potential enrollees.
- Maintains detailed records of submissions and state responses.
- Travels between Elder Service Plan sites and participant homes to perform duties.
- Prepares and submits assessments for state review and follows up as needed.
- Manages medical records documentation and EMR entries for enrollment.
