Job Profile Summary

This role focuses on providing pharmacy related services in a hospital or retail setting. This is a fulltime position, in Inpatient Pharmacy.

Hours are variable.

Job Overview

Under general supervision of a pharmacist, this technician works at gathering patient medication history assessments for new admissions to the hospital. Responsibilities include verifying patient's home medication list and ensuring it is complete and accurate. Communicates to the pharmacist any discrepancies or concerns regarding medication regimens for review and intervention and maintaining detailed documentation.

Duties and Responsibilities: The duties and responsibilities listedbelow are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.

Gathering patient medication history:
Perform medication history assessments for new admissions to the hospital, providing complete information regarding each medication identified (i.e., drug, dose, route, frequency, indication for PRN, and last dose taken-date/time).
Verify patient allergy information and ensure complete documentation (i.e., substance, allergy category, reaction description)


Data entry and verification:
Accurately entering patient medication information into the pharmacy system, including dosage, frequency, and route of administration.


Identifying potential issues:
Comparing the patient's current medication list to new prescriptions or orders, flagging potential problems like drug interactions, duplications, allergies, or missed medications.


Communicating findings to pharmacist:
Reporting any discrepancies or concerns regarding the patient's medication regimen to the pharmacist for review and necessary interventions.


Documentation:
Maintaining detailed documentation of the medication reconciliation process, including sources of information and any identified issues.


Required skills and qualifications:
Pharmacy technician license: Must hold a valid pharmacy technician license in the state of practice.
Attention to detail: High level of accuracy when collecting and verifying medication information.
Communication skills: Effective communication with patients, caregivers, and healthcare providers to gather accurate medication history.
Basic medical knowledge: Understanding of medical terminology, drug names, and potential drug interactions.
Computer proficiency: Ability to navigate pharmacy software and electronic health records. Conducts interpersonal relationships with patients, visitors, physicians and fellow employees in a courteous and customer friendly manner

Minimum Qualifications:

1. High school diploma or equivalent.

2. Certified Pharmacy Technician (CPhT).

3. Massachusetts Pharmacy Technician Registration.

4. Two (2) years of pharmacy-related experience.

Preferred Qualifications:

1. Associate's degree.

2. Three (3) years of pharmacy-related experience in a hospital setting.

3. Previous retail pharmacy experience including prescription transcription duties preferred. Trained in general use of pharmacy computer systems preferred.

4. Strong mathematical skills.

Physical Requirements:

1. Ability to walk and stand 90% of the day and to occasionally lift up to 30 pounds.

2. Frequent reaching and grasping below, at, and above shoulder level.

3. Pushing and/or pushing non-motorized equipment (e.g. carts, IV rolling rack, and medication rolling racks, etc.) weighing 40 to 60 pounds.

4. Requires manual dexterity using fine hand manipulation to operate prescription medications and computer keyboard

5. Requires ability to see medications, computer screen and reports.

6. Reading automated dispensing reports regularly.

7. Works in an area with some discomfort due to dust, dirt, and temperature changes.

8. May be exposed to broken glass and some exposure to hazardous chemicals.
