Witham Health Services is seeking a Medication Reconciliation Technician to ensure patient safety by obtaining accurate medication histories in the Emergency Department. The role involves clerical and clinical tasks, reporting to the Pharmacy Director. Candidates should have a high school diploma and preferably pharmacy technician certification.

The Medication Reconciliation Technician (MRT) will independently obtain medication and allergy histories for patients in the Emergency Department (ED). Responsibilities include interviewing patients, families, calling local pharmacies, Long Term Care facilities, and other resources. The information will be entered into a computer system for use by healthcare providers to determine treatment plans and medication orders during and after hospitalization. The goal is to create a complete and accurate medication list promptly, promoting patient safety by reviewing medications to ensure correctness in type and dosage. The MRT assists in minimizing medication discrepancies during hospital admission and provides the most up-to-date medication information.
The MRT reports to the Pharmacy Director or designee. Program oversight, continuing education, and policies are supported by the Pharmacy Director.
The role involves clerical, clinical tasks, and other duties as assigned.
