Job Overview

The LPN Medication Care Manager is responsible for providing the highest quality of care by administering medication and treatments safely and organized. Responsibilities include medication administration, documentation, conducting SHUs, and providing resident care, all in accordance with laws, standards, regulations, and Sunrise policies to ensure quality care.

Responsibilities & Qualifications

Responsibilities:

• Administer medications and treatments safely and organized.
• Review and document pertinent resident information.
• Receive medication updates from RCD or Wellness Nurse.
• Administer and observe medications using the Six Rights of Medication Pass.
• Ensure timely medication passing and proper documentation.
• Maintain confidentiality of resident information.
• Report resident concerns during medication administration.
• Restock medication carts and assist with medication checks.
• Assess residents for 'as needed' medications and document accordingly.
• Count narcotics with another MCM or LCM each shift.
• Maintain and clean medication and treatment areas.

Resident Care:

• Notify RCD of concerns.
• Participate in meetings and care plan development.

Risk Management & Safety:

• Ensure compliance with safety and OSHA regulations.
• Practice safety procedures including PPE and hazard reporting.

Training & Team Contribution:

• Participate as a team member and in engagement programs.
• Attend meetings and training sessions.
• May supervise other care managers.

Qualifications:

• Licensed as LPN, LVN, or equivalent.
• Maintain required certifications (CPR, First Aid).
• At least 18 years old; experience with seniors preferred.
• Effective communication and judgment skills.
