In Med History, our goal is to obtain the best possible medication history for each patient admitted to the hospital. This stepis crucial to completion of Joint Commission accreditation-mandated medication reconciliation. Medication History Technicians claim patients from a work list based on the patient’s location and the tech's scheduled shift assignment. Technicians follow a structuredinterview with the patient or caregiver and document theinformationinto the electronic medical record. Discrepancies are communicated directly to the provider/pharmacist for resolution.
