Position Summary / Career Interest:
The Medication History Technician (MHT) team serves our inpatient locations at main campus in Kansas City, KS, and supports the accurate and timely completion of medication histories for our patients. This critical role helps ensure our pharmacists and providers know what medications patients were taking prior to admission to the hospital to provide care to the patient during the stay. Additionally, this information allows our teams to provide patients with clear medication instructions at discharge. The MHTs obtain this information through a variety of means including interviewing patients, obtaining medication lists from other medical facilities, contacting retail pharmacies, and interviewing family members or caregivers. This information is then documented in the electronic health record.
