Job summary

What We're Looking For
  • High School Diploma or equivalent (required)
  • Minimum two (2) years of pharmacy technician experience in a retail, hospital, or health‑system setting (required)
  • Preferred: three (3) years of pharmacy technician experience in a hospital environment
  • National PTCB Certification (CPhT) (required)
  • State of Louisiana Pharmacy Technician License (required)
  • Pharmacy Technician Career Ladder (Assessment Certificates) required within 60 days of hire
  • Strong attention to detail with a focus on accuracy and quality
  • Working knowledge of brand and generic drug names, dosing, uses, and formulations
  • Strong prioritization skills with the ability to work independently and manage competing demands
  • Excellent customer‑service, communication, and interpersonal skills
  • Basic computer proficiency, including Microsoft Office applications
  • Ability to adapt to a fast‑paced and changing healthcare environment

What You Will Do

You will obtain complete, accurate, and timely medication histories for patients admitted through the Emergency Department, newly admitted inpatients, and pre‑operative patients to support safe medication reconciliation.

  • Obtain comprehensive medication histories using patient/caregiver interviews, retail pharmacy records, MARs, and third‑party software.
  • Conduct in‑person and telephone interviews for ED, inpatient, and pre‑operative patients.
  • Collect prescription, OTC, herbal, and supplement information including dose, route, frequency, and last taken time.
  • Ensure timely completion of medication histories for physician review.
  • Accurately document medication histories in the electronic health record.
  • Communicate with external pharmacies, physicians, and insurance companies as needed.
  • Coordinate with nurses, pharmacists, physicians, and case managers to identify high‑priority patients.
  • Resolve medication history discrepancies and elevate to pharmacists or providers as needed.
  • Conduct medication history quality audits.
  • Assist with Emergency Department medication distribution as needed.
  • Perform monthly unit inspections and audit rounds in the Emergency Department.
  • Prioritize workload effectively to meet productivity expectations.
  • Perform all other duties as assigned.
