Job summary

Baton Rouge General Medical Center is seeking a licensed pharmacist to join our hospital-based pharmacy team. You will provide safe, accurate medication therapy management, fulfill physician orders, monitor drug therapy, and support medication safety.

The ideal candidate will hold a BS in Pharmacy, have a Louisiana license, and demonstrate strong clinical judgment, communication, and collaboration with clinicians and nursing staff.

Qualifications

  • Candidate must hold a BS in Pharmacy.
  • Licensed to practice pharmacy in Louisiana.
  • Hospital pharmacy experience preferred.
  • Strong knowledge of drug therapy, contraindications, interactions, and USP sterile standards.
  • Ability to supervise pharmacy technicians and support personnel.
  • Effective communication, clinical judgment, and problem-solving skills.
  • Ability to interpret medication orders and collaborate with clinical teams.

Responsibilities

  • Provide safe, accurate, and cost-effective medication therapy management across the hospital.
  • Prepare and dispense medications according to physician orders and protocols.
  • Interpret, verify, and document written and electronic medication orders.
  • Maintain accurate patient medication profiles.
  • Compound sterile products, chemotherapy, and TPN per USP standards.
  • Issue and document controlled substances per regulations.
  • Monitor therapies for contraindications, interactions, allergies, and appropriateness.
  • Review patient charts and document clinical interventions.
  • Identify and report adverse drug reactions.
  • Consult with practitioners and recommend cost-effective therapeutic alternatives.
  • Participate in medication use reviews, audits, and quality improvement.
  • Supervise pharmacy technicians and support personnel.
  • Perform medication storage inspections and remove outdated medications.
  • Support pharmacy automation and technology systems.
  • Collaborate with IS, vendors, Nursing, Billing, and Clinical Informatics.
  • Maintain licensure, CE documentation, competency requirements, and attend meetings.
  • Perform all other duties as assigned.
