Job summary

Magee Rehabilitation Hospital is seeking a Medication History Pharmacy Tech to accurately collect and enter current patient medications into the hospital system, supporting physicians and admitting/discharging processes. The role includes patient interviews, BPMH reconciliation, and coordinating with pharmacists to resolve discrepancies.

Under supervision, you will perform limited medication management tasks and ensure timely completion of duties within department policies and timelines,

Qualifications

• Requires High School Diploma or GED.
• Pharmacy Technician Certification (CPhT) and Pennsylvania Registered Pharmacy Technician (RPT) are preferred.
• Excellent written and verbal communication; proficient with basic PC skills.

Responsibilities

• Interview patients to collect medication histories and construct pre-admission lists (PAML).
• Reconcile and document BPMH in the medical record.
• Communicate with outside pharmacists or physicians to resolve discrepancies.
• Assist with medication management and distribution under supervision of a pharmacist.
• Contribute to department projects and adhere to policies and timelines.
