Magee Rehabilitation Hospital is seeking a Medication History Pharmacy Tech to collect payer and patient medication histories and reconcile BPMH for admitted and discharged patients.

You will interview patients, review internal and external sources, and enter data into the hospital information system. Under pharmacist supervision, you may perform limited medication management and distribution tasks.

Formación

• Must have excellent written and verbal communication skills.
• Proficient with basic PC skills and medication information.
• Knowledge of medications, strengths, and indications.

Responsabilidades

• Collect and record patient medications and reconcile BPMH.
• Communicate with patients, families, and providers to gather history.
• Enter data into the hospital information system accurately.
• Coordinate with pharmacists to resolve discrepancies.
• Perform limited medication management under supervision of a pharmacist.
