Job summary
Jefferson Health in Philadelphia seeks a Medication History Pharmacy Tech to collect and enter current patient medications into the hospital system. You will perform patient interviews, reconcile BPMH, and coordinate with pharmacists to resolve discrepancies, while handling patients’ own meds under supervision.
Minimum qualifications include a high school diploma and 1 year of technician experience; CPhT/RPT certifications are preferred.
Qualifications
- Minimum qualification is a high school diploma or GED.
- At least 1 year of Pharmacy Technician experience required.
- Proficient with basic PC skills and strong communication abilities.
- Certification as a Pharmacy Technician (CPhT) and/or RPT is preferred.
Responsibilities
- Meets all standards of patient satisfaction criteria.
- Interview patients and construct a pre-admission medication list (PAML).
- Reconcile BPMH accurately in the medical record.
- Contact outside pharmacists or physicians to resolve discrepancies.
- Handle patients’ own medications securely and document properly.
- Set priorities and assist with medication delivery as time allows.
- Collect prescription benefits information and support timely delivery.
- Participate in committees and department projects.
