Job summary

Northwest Hospital, part of LifeBridge Health, seeks a Medication History Technician to obtain accurate, comprehensive medication histories from patients through interviews, verify with outside sources for all active medications, and document for physician reconciliation.

Qualifications

3–5 years related experience in medication history.
Licensed Pharmacy Technician with required certifications.
Strong interviewing and documentation skills.

Responsibilities

Obtain accurate, comprehensive medication history from patients through interviews.
Communicate with outside sources to verify all active medications.
Document current medication history for physician reconciliation.
