Job summary

Baptist Hospitals of Southeast Texas is seeking a Medication History Pharmacy Technician to obtain comprehensive medication histories from patients and families, document them in the EMR, and support medication safety initiatives. You will collaborate with pharmacists and outpatient providers to resolve discrepancies and maintain high-quality records.

Responsibilities include interviewing patients, reviewing records, updating the EMR, and assisting with inventory and quality improvement efforts

Qualifications

• Obtain comprehensive medication history from patients and families.
• Identify true allergies and resolve discrepancies with care teams.
• Enter medication history accurately into the EMR/pharmacy system.
• Support quality improvement initiatives and medication safety efforts.
• Collaborate with pharmacists and outpatient providers as needed.

Responsibilities

• Conduct patient interviews to gather home medication lists (OTC, supplements, Rx).
• Review records and confirm medication details with outpatient pharmacies.
• Input histories into EMR/pharmacy system accurately and timely.
• Identify discrepancies and communicate them to the pharmacist for resolution.
• Assist with cycle counts and inventory management in the department.
