Job summary

Penn Medicine seeks a Medication History Specialist to obtain and document accurate medication histories for admitted patients. Under a licensed pharmacist's supervision, you will interview patients, families, and caregivers and reconcile lists in the EHR.

You will collaborate with pharmacists, providers, and nursing staff to support safe medication use, verify information from pharmacies and records, and uphold HIPAA and hospital policies.

Qualifications

• Structured patient and caregiver interviews to obtain a complete medication history.
• Accurate documentation of drug names, doses, routes, and frequencies in the EHR.
• HIPAA compliance and high attention to detail in all records.

Responsibilities

• Interview patients and caregivers to collect complete medication histories (prescriptions, OTCs, herbs, supplements).
• Document medications accurately in the EHR with doses, routes, and last doses.
• Verify information from pharmacies and previous records; reconcile discrepancies.
• Flag safety issues or incomplete data for pharmacist/clinician review.
• Collaborate with pharmacists, nurses, and physicians to ensure accurate admission medication reconciliation.
