Job summary

MercyOne North Iowa Medical Center in Mason City is seeking a Medication Historian Pharmacy Tech to collect and verify admission medication histories. You will interview patients to build accurate lists, document medications, and ensure patient safety from the first contact.

The role requires pharmacy technician registration, national certification, and BLS certification, with a focus on teamwork and compliance in a hospital setting.

Qualifications

• Obtain an accurate admission medication list via face-to-face interview including prescriptions, OTCs and herbs.
• Document allergies and reactions and coordinate with care teams for accuracy.

Join the MercyOne Family! We are looking to hire a Medication Historian Pharmacy Tech.

As a Medication Historian Pharmacy Tech at MercyOne, assists with the accurate and complete collection of medication history data in a variety of hospital settings. This role prioritizes the process of obtaining an accurate and complete medication list upon hospital admission to ensure patient safety.

Schedule:

• 8/hour rotating shifts
  

General Requirements:

• Obtains an accurate and complete admission medication list via a face-to-face interview with patients to include prescription, over the counter, and herbal products noting drug name, dose, frequency, compliance, and last dose taken. Also obtains and documents any known drug allergies and reactions. This may require collaboration with alternative information sources such as family, physician office, nursing home, or retail pharmacy.
• Communicates effectively and uses advanced interviewing skills to conduct the medication history interview with patients in a manner that is professional and non-invasive.
• Problem solves and applies critical thinking skills in obtaining admission medications lists for a wide variety of patient populations.
• Applies advanced computer skills to gather information and document the patient’s admission medication list.
• Knows and adheres to all laws and regulations pertaining to patient health, safety, and medical information.

Education:

• A high school diploma or equivalent is required.
• Must be a registered pharmacy technician or be eligible for technician/trainee registration in the State of Iowa. This must be completed prior to start of employment.
• National Pharmacy Technician Certification required within 6 months of employment.
• Preferred Experience:
• o Two or more years of experience as a pharmacy technician (retail preferred) or allied health professional, with knowledge of medications including brand/generic names, available dosages, frequencies and indications.
• o Completes the following mandatory courses
• Must provide proof of Basic Life Support (BLS Healthcare Provider, American Heart Association; or American Red Cross, Professional Rescuer-LIVE class only) via current card prior to employment or within 60 days of hire and maintains current status.
• Completes Mandatory Reporting: Child and Dependent Adult Abuse requirement within 6 months of hire and renews this requirement every 5 years.
