Job summary

MercyOne Des Moines is seeking a Medication History Pharmacy Technician to obtain, verify, and record initial medication histories from patients and families at presentation to Mercy patient care areas.

The role includes interviewing patients/families, compiling current medication lists, allergies, and relaying information to pharmacists for unresolved items. Evening shift 10:30 AM–7:00 PM; Iowa licensure required.

Qualifications

• High School Diploma or GED required.
• National Pharmacy Technician Certification and Iowa Board registration at hire.
• 2,000 hours of pharmacy technician experience required.

Responsibilities

• Interview patients and designated family/caregivers to obtain current medications.
• Record complete medication lists in the EMR, including prescriptions, OTC meds, vitamins, and supplements.
• Document allergies and clarify reaction details in the EMR.
• Consult with a pharmacist on uncertainties related to medication and allergy history.
• Perform other Iowa pharmacy technician duties within state law.

Des Moines, Iowa (IA), MercyOne Des Moines Join the MercyOne Family! We are looking to hire a Medication History Pharm Tech. As a Medication History Pharm Tech at MercyOne, will provide technical support in obtaining, verifying for accuracy, and recording initial medication histories from patients &/or their designated family/caregivers upon presentation to Mercy patient care areas. Includes conducting patient &/or family interviews focused on obtaining current medication regimens including prescription and over-the-counter (non-prescription) medications, as well as vitamin and herbal supplements. Perform other functions of a pharmacy technician registered in the State of Iowa and within the scope of Iowa law as needed. Schedule: 8-hours Evenings 10:30 AM - 7:00 PM

General Requirements:

• Successfully complete Medication History Pharmacy Technician Competency/Skill Set.
• Interview patients &/or their designated family/caregivers as soon as possible after patient presents for care to MercyOne clinical areas (includes the Emergency Department, as well as inpatient and outpatient areas).
• Obtain & record in the electronic medical record a current list of medications to include: prescription medications, over-the-counter (non-prescription) medications, vitamin and nutritional supplements, as well as the source(s) for this information.
• Recording should include the following: name/location of patient's ambulatory pharmacy, medication/supplement name, dosage form, strength, dose, day/time of most recent dose, and source(s) of the information being recorded.
• Attempt to clarify as many uncertainties and compile a Best Possible Medication History utilizing patient and family/caregiver information, ambulatory pharmacy records, clinic records, long-term-care facility records and information from previous hospital encounters.
• Obtain & record in the electronic medical record an allergy history for each patient.
• In doing so, clarify & record the nature of the allergic reaction when necessary.
• Consult with a pharmacist in all cases of unresolved uncertainties related to a patient’s medication & allergy history.
• Perform other functions of a pharmacy technician registered in the State of Iowa and within the scope of Iowa law as needed.

Education:

• High School Diploma or GED
• National Pharmacy Technician Certification and Registered with the Iowa Board of Pharmacy at the time of hire OR Registered as a Pharmacist-Intern with the State of Iowa at time of hire
• 2,000 hours of pharmacy technician experience
