We are looking to hire a Medication Historian Pharmacy Tech. As a Medication Historian Pharmacy Tech at MercyOne, assists with the accurate and complete collection of medication history data in a variety of hospital settings. This role prioritizes the process of obtaining an accurate and complete medication list upon hospital admission to ensure patient safety.
8/hour rotating shifts
Obtains an accurate and complete admission medication list via a face-to-face interview with patients to include prescription, over the counter, and herbal products noting drug name, dose, frequency, compliance, and last dose taken. Also obtains and documents any known drug allergies and reactions. This may require collaboration with alternative information sources such as family, physician office, nursing home, or retail pharmacy. Communicates effectively and uses advanced interviewing skills to conduct the medication history interview with patients in a manner that is professional and non-invasive. Problem solves and applies critical thinking skills in obtaining admission medications lists for a wide variety of patient populations. Applies advanced computer skills to gather information and document the patient’s admission medication list. Knows and adheres to all laws and regulations pertaining to patient health, safety, and medical information.
A high school diploma or equivalent is required. Must be a registered pharmacy technician or be eligible for technician/trainee registration in the State of Iowa. This must be completed prior to start of employment. National Pharmacy Technician Certification required within 6 months of employment.
