Job summary

Community Health Centers of the Central Coast is seeking a Medication Case Manager to assist patients in obtaining and using essential medications, under the supervision of the Health Center Manager and Clinical Services Supervisor.

The role emphasizes patient-centered care, HIPAA compliance, high-volume coordination with care teams, data entry, and adherence to medication guidelines. A high school diploma or GED and knowledge of medications are required; EHR experience is preferred.

Qualifications

• High school diploma or GED required.
• Previous pharmacy/clinic experience preferred.
• Knowledge of medications and their uses.
• Ability to read, analyze, and interpret safety rules and procedures.
• Ability to write routine reports and communicate clearly.

Responsibilities

• Reviews prescriptions to determine potential financial coverage and prior authorization.
• Enters data such as patient name, prescribed medication, cost, income level and eligibility for assistance programs.
• Assists patients in finding links to health resources and programs.
• Ensures patients follow up with their primary provider for medication reviews.
• Communicates with care teams to document medication renewals and refills.
• Manages inventory of medication samples and processes refill requests.
• Reviews and loads refill requests with pertinent information for providers.
• Ensures lab tests for prescribed medications adhere to guidelines where applicable.
• Adds diagnoses to medication requests when present in patient records.
• Notifies patients when medications are ready for pickup.
• Maintains HIPAA and confidentiality across patient interactions.
