Job summary

Community Health Centers of the Central Coast, Inc. is seeking a Medication Case Manager to support patients in obtaining and using essential medications under supervision. The role partners with the Medical Department to review prescriptions, assist with medication access, document care, and coordinate with providers.

Qualifications

• High school diploma or GED required.
• Previous experience working in a pharmacy or clinic setting preferred.
• Must have knowledge of medications and their uses.

Responsibilities

• Review prescriptions to determine potential sources for financial coverage and prior authorization.
• Enter data such as patient name, prescribed medication, cost, income level and eligibility for medication assistance programs.
• Assist patients in linking to health resources and programs.
• Ensure patients follow up with their primary provider at least once a year.
