Advocate Health is seeking a registered nurse to conduct Medicare Wellness Visits in a 40-hour work week, Monday–Friday, 8–5, in the Charlotte area. The RN will perform preventive assessments, document findings, and coordinate preventive services.

Responsibilities include education, care planning, and collaboration with the care team to close gaps in care, with opportunities for professional development and competitive pay.

Competenze

• Graduate from an accredited School of Nursing.
• BSN preferred and current RN license.
• BLS certification required per policy.
• 2 years of clinical specialty experience preferred; certification within 2 years if not currently certified.

Mansioni

• Conduct RN-led Medicare Wellness Visits with preventive care assessments and Medicare components.
• Document comprehensive patient assessments including health history, medications, and functional status.
• Perform and document depression, cognitive, fall risk, functional, and preventive health screenings.
• Identify and close preventive care gaps such as immunizations and screenings.
• Provide patient education, health coaching, motivational interviewing, and self-management support.
• Develop personalized prevention plans with goals and follow-up needs.
• Escalate abnormal findings to providers per protocol.
• Coordinate referrals and follow-ups with providers and interdisciplinary teams.
• Maintain accurate EHR documentation compliant with Medicare, CMS and coding rules.
• Engage in quality improvement and workflow optimization to improve patient outcomes.
