Job summary

Advocate Health is seeking an RN to conduct Medicare Wellness Visits and document comprehensive assessments. You will deliver preventive screenings, health coaching, and personalized prevention plans, while coordinating referrals with an interdisciplinary team.

The role requires an accredited nursing degree, current RN license, and BLS; BSN preferred and NP licensure may be applicable depending on state rules. Monday to Friday, 8–5, 40 hours weekly.

Qualifications

• Graduate from an accredited School of Nursing.
• BSN preferred.
• Current RN license in the work state; NP license if applicable.
• BLS required; experience in clinical specialty preferred.

Responsibilities

• Conduct RN-led Medicare Wellness Visits with preventive care components.
• Document comprehensive health assessments and health history.
• Perform required screenings and close preventive care gaps.
• Provide patient education and self-management coaching.
• Coordinate referrals and follow-up with care teams.
• Maintain accurate EHR documentation per policy.
