gobravvo in Portsmouth, VA is seeking a dedicated Nurse Practitioner (NP) to join our healthcare team. This full-time role centers on value-based Medicare patient care within a collaborative setting, emphasizing preventive care and coordinated treatment.

You will work with a multidisciplinary team, utilizing on-site labs and EMR systems to support quality outcomes. The position offers comprehensive benefits and opportunities for professional growth.

Qualifications

• Must hold MSN, DNP or NP certificate from an accredited program.
• Active, unrestricted NP licensure in the state and a current DEA registration.
• National NP certification in Family, Adult-Gerontology, Adult or Gerontological tracks.

Responsibilities

• Conduct patient assessments and routine visits approximately every 3 months.
• Manage a daily patient load of 13–16 with 20–40 minute visits.
• Provide preventive, acute and chronic disease management.
• Collaborate with physicians, scribes, medical assistants and social workers.
• Document care using Canopy and Greenway EMR systems.
• Coordinate referrals via RubiconMD and engage in community programs.
• Support value‑based care and ensure regulatory/documentation compliance.
