Job summary

Northwell Health is seeking an Intake Navigator III to support Medicare and Medicaid eligibility, enrollment, and entitlement processes for our client providing home healthcare services in the New York metropolitan area. You will handle paperwork, resolve eligibility issues, assist with reporting, and ensure compliance with state and federal regulations.

Qualifications

• Bachelor’s degree preferred.
• 2–3 years of experience with Medicaid, Medicare, MLTC, MAP, D-SNP, PACE, or related social services/government work.
• Strong knowledge of eligibility and entitlement rules.
• Excellent communication, organization, and computer skills.
• Experience with Care Compass, E-Paces, MARX, or Tableau preferred.

Responsibilities

• Manage Medicare/Medicaid eligibility and entitlement issues
• Review and upload enrollment paperwork
• Work with Social Work, Finance, and other departments
• Verify eligibility in systems such as E-Paces and MARX
• Resolve disenrollment, denials, appeals, and Medicaid loss issues
• Prepare reports and support compliance efforts
• Liaise with agencies such as HRA, LDSS, NYSDOH, and CMS
