Job summary

Rockport Healthcare Services is seeking a Medicare/MDS Coordinator to lead the interdisciplinary team in completing MDS assessments in compliance with state and federal regulations. The role reports to the Director of Nursing Services and focuses on coordinating care, documentation, and nursing interventions as needed.

Ideal candidates are RNs with strong communication and organizational skills, CPR certification preferred, and two years of long-term care nursing experience.

Qualifications

• Current licensure in nursing (RN) is required.
• Written and verbal communications in English are essential for business.
• Administrative and organizational abilities are required.

Responsibilities

• Coordinate the Medicare/MDS resident assessment process.
• Ensure the Interdisciplinary Team completes MDS assessments timely.
• Coordinate development, implementation and evaluation of the plan of care.
• Administer or implement treatments, medications, or nursing interventions as ordered.
• Provide nursing care per infection control standards and center policies.
• Follow safety policies in nursing care.
• Initiate emergency measures per center policy and nursing practice standards.
