Job summary

Rockport Healthcare Services is seeking a Medicare MDS Coordinator to oversee the IDT-driven MDS assessment process and ensure timely completion in accordance with state and federal regulations.

The role requires RN licensure, strong communication, and administrative skills, with preferred CPR certification and LTC nursing experience. On-site coordination in Roseville, CA supports quality resident care and regulatory compliance.

Qualifications

• RN license required.
• English communication skills required.
• Administrative and organizational abilities required.
• CPR certification preferred.
• Two years nursing experience in long-term care preferred.
• Supervisory experience preferred.

Responsibilities

• Coordinate the Medicare/MDS resident assessment process.
• Ensure IDT completes the MDS assessment in a timely manner.
• Coordinate development, implementation and evaluation of the care plan.
• Coordinate and perform treatments, medications or other nursing interventions as indicated by the plan of care or orders.
• Provide nursing care in accordance with infection control standards.
• Follow safety policies in nursing care.
• Initiate emergency measures per center policy and nursing standards.
• Ensure exchange and use of essential information for quality resident care.
• Maintain documentation per Federal and State regulations and Company policy.
• Coordinate and/or participate in all assigned meetings and inservices.
• Present a professional image to consumers and resolve consumer concerns.
• Protect residents’ rights.
