Qualifications

Responsibilities

• Manage a contract panel of chronic patients, monitoring chemotherapy administration numbers.
• Coordinate discharge planning with case management, social work, and families in Springfield.
• Collaborate with respiratory, physical therapy, and pharmacy to maintain the contract care plan.
• Update care-team contacts so families in Springfield, MA know who to ask.
• Handoff at shift change using SBAR to avoid gaps for the next Medical Technologist.
• Triage incoming patients and prioritize cases by acuity.
