Job summary

CommonSpirit Health at Home is seeking a Medical Social Worker - Hospice to support patients and families in aligning care with the physician's plan, provide crisis intervention, and help navigate social, emotional and economic barriers to healing.

The role requires a Master’s in Social Work and active state licensure, plus CPR/BLS certification and 1 year of healthcare experience. Benefits include bonus, generous vacation, medical/dental/vision, tuition reimbursement, and a Fidelity 401(k) plan.

Qualifications

  • Master’s degree from an accredited school approved by the Council on Social Work Education.
  • Currently licensed to practice in the state served.

Responsibilities

  • Establishing a care plan in accordance with the physician’s plan of treatment.
  • Providing crisis intervention as required.
  • Assisting the physician and other caregivers in understanding social, emotional and economic factors impacting medical improvement.
  • Providing patient care education to patients and caregivers.
  • Assisting in making appropriate community referrals for public assistance, housing, transportation, and other resources.
  • Collaborating with caregivers to resolve issues affecting the caregiving relationship.
  • Completing and submitting all required documentation accurately and timely.

Job Summary and Responsibilities

At VNA Health at Home, we strive to embody our mission of delivering hope and healing to those we serve. As a Medical Social Worker - Hospice, you’ll engage in a range of exciting responsibilities, including:

  • Establishing a care plan in accordance with the physician’s plan of treatment.
  • Providing crisis intervention as required.
  • Assisting the physician and other caregivers in understanding the significant social, emotional and economic factors influencing the patient’s ability to improve medically.
  • Providing patient care education to patients and caregivers.
  • Assisting in making appropriate community referrals to gain access to additional resources as needed (i.e. applications for public assistance, housing problems, additional health care resources, transportation)
  • Working with the patient’s caregivers to resolve problems interfering with an optimum caring relationship between the patient and caregiver.
  • Completing and submitting all required documentation in an accurate and timely manner.
Job Requirements

Qualifications:

  • Master’s degree from an accredited school approved by the Council on Social Work Education. In Indiana, must also be certified under IC25-23.6-4-3.
  • Currently licensed to practice in the state serviced.
  • One year social work experience in a health care setting.
  • Current CPR/BLS certification through American Heart Association or American Red Cross and compliance with state requirements
