Job summary

CommonSpirit Health at Home is seeking a Medical Social Worker to develop care plans aligned with physician treatment, provide crisis intervention, and support patients and families with social, emotional, and economic factors affecting care. The role includes referrals to community resources, documentation, and coordination with caregivers.

Qualifications include a CSWE-accredited MSW, active state licensure, at least one year in a health care setting, and CPR/BLS certification.

Qualifications

• Master’s degree from an accredited school approved by CSWE.
• Currently licensed to practice in the state serviced.
• One year social work experience in a health care setting.
• CPR/BLS certification through American Heart Association or American Red Cross and compliance with state requirements

• Working with the patient’s caregivers to resolve problems interfering with an optimum caring relationship between the patient and caregiver.
• Completing and submitting all required documentation in an accurate and timely manner.
