Job summary

CommonSpirit Health at Home is seeking a Medical Social Worker to develop care plans, provide crisis intervention, and connect patients with community resources in a home health setting.

The role requires an MSW, state licensure, and at least one year of health care social work experience, with CPR/BLS certification maintained.

Qualifications

• Master’s degree from an accredited school approved by the Council on Social Work Education.
• Licensed to practice in the state serviced.
• One year social work experience in a health care setting.
• Current CPR/BLS certification through American Heart Association or American Red Cross.

Responsibilities

• Establishing a care plan in accordance with the physician’s plan of treatment.
• Providing crisis intervention as required.
• Assisting in understanding social, emotional and economic factors affecting medical improvement.
• Providing patient care education to patients and caregivers.
• Assisting in making appropriate community referrals for additional resources (public assistance, housing, transportation).
• Working with caregivers to resolve problems affecting the patient-caregiver relationship.
• Completing and submitting all required documentation accurately and timely.

Responsibilities

At St. Elizabeth's Health at Home, we strive to embody our mission of delivering hope and healing to those we serve. As a Medical Social Worker, you’ll engage in a range of exciting responsibilities, including:

• Establishing a care plan in accordance with the physician’s plan of treatment.
• Providing crisis intervention as required.
• Assisting the physician and other caregivers in understanding the significant social, emotional and economic factors influencing the patient’s ability to improve medically.
• Providing patient care education to patients and caregivers.
• Assisting in making appropriate community referrals to gain access to additional resources as needed (i.e. applications for public assistance, housing problems, additional health care resources, transportation)
• Working with the patient’s caregivers to resolve problems interfering with an optimum caring relationship between the patient and caregiver.
• Completing and submitting all required documentation in an accurate and timely manner.

Qualifications

• Master’s degree from an accredited school approved by the Council on Social Work Education. In Indiana, must also be certified under IC25-23.6-4-3.
• Currently licensed to practice in the state serviced.
• One year social work experience in a health care setting.
• Current CPR/BLS certification through American Heart Association or American Red Cross and compliance with state requirements

Pay Range

$28.10 - $41.80/hour
