Job summary

VNA Health is seeking a Medical Social Worker to implement standards of care for social work services within our home health program. The role emphasizes compassionate support, collaboration with clinical teams, and adherence to regulatory requirements to improve patient outcomes.

You will assess psychosocial needs, document evaluations, coordinate services with community resources, and assist families in navigating care.

Qualifications

• Master's Degree in social work from an accredited school.
• Minimum of one year of healthcare experience.
• Home health experience preferred.
• Licensed driver with insured automobile.
• Demonstrates good verbal and written communication, and organization skills.

Responsibilities

• Assess psychosocial status related to the patient’s illness and environment.
• Carry out social evaluations and plan interventions.
• Maintain clinical records for social work referrals.
• Provide information and referral services for patients and families.
• Serve as liaison between patients/families and community agencies.
• Collaborate with organization personnel to support patient care.
• Develop contracts with public/private agencies as resources.
• Participate in care planning and case conferences.
• Support physicians and other team members on health factors.

Job Summary

The Medical Social Worker is responsible for the implementation of standards of care for medical social work services. Respects and promotes the mission, vision, and values of VNHC.

Job Duties:

• Assesses the psychosocial status of patients related to the patient’s illness and environment.
• Carries out social evaluations and plans intervention based on evaluation findings.
• Maintains clinical records on all patients referred to social work.
• Provides information and referral services for Organization patients and families/caregivers regarding practical and environmental needs.
• Provides information to patients or families/caregivers and community agencies.
• Serves as liaison between patients or families/caregivers and community agencies.
• Maintains collaborative relationships with Organization personnel to support patient care.
• Maintains and develops contracts with public and private agencies as resources for patient and organization personnel.
• Participates in the development of the total plan of care and case conferences as required.
• Assists physician and other team members in understanding significant social and emotional factors related to health problems.
• Meets the average daily productivity standards as set by the agency.
• Knowledge of state, federal, local and accreditation regulations pertaining to the delivery of home health services.
• Other duties as delegated by the Director of Home Health/Clinical Supervisor.
