Job summary

CommonSpirit Health at Home is seeking a Medical Social Worker to support patients in a home-based care setting. You will develop care plans, address social and economic barriers, and coordinate with physicians and caregivers to promote positive outcomes.

The role requires a Master’s in Social Work, Indiana certification, and at least 1 year of health-care social work experience, with current CPR/BLS certification. This is an on-site position in Indiana with competitive pay.

Qualifications

• Master’s degree in Social Work from an accredited school; Indiana certification required.
• Current state licensure to practice social work.
• At least 1 year of social work experience in a health-care setting.
• CPR/BLS certification through AHA or Red Cross; comply with state requirements.

Responsibilities

• Establish a care plan aligned with physician plan of treatment.
• Provide crisis intervention as needed.
• Assist in understanding social, emotional, and economic factors affecting patient care.
• Educate patients and caregivers on care plans.
• Facilitate community referrals for public assistance, housing, transportation, etc.
• Coordinate with caregivers to ensure optimal patient-caregiver relationships.
• Complete and submit documentation accurately and timely.

Job Summary and Responsibilities

At American Nursing Care Health at Home, we strive to embody our mission of delivering hope and healing to those we serve. As a Medical Social Worker , you’ll engage in a range of exciting responsibilities, including:

• Establishing a care plan in accordance with the physician’s plan of treatment.

• Providing crisis intervention as required.

• Assisting the physician and other caregivers in understanding the significant social, emotional and economic factors influencing the patient’s ability to improve medically.

• Providing patient care education to patients and caregivers.

• Assisting in making appropriate community referrals to gain access to additional resources as needed (i.e. applications for public assistance, housing problems, additional health care resources, transportation)

• Working with the patient’s caregivers to resolve problems interfering with an optimum caring relationship between the patient and caregiver.

• Completing and submitting all required documentation in an accurate and timely manner.

Job Requirements

Qualifications:

• Master’s degree from an accredited school approved by the Council on Social Work Education. In Indiana, must also be certified under IC25-23.6-4-3.

• Currently licensed to practice in the state serviced.

• One year social work experience in a health care setting.

• Current CPR/BLS certification through American Heart Association or American Red Cross and compliance with state requirements

Pay Range

$26.70 - $39.71/hour
