Qualifications
- Master of Social Work from an accredited program (CSWE).
- License in the state where Oak Street Health Center is located.
- US work authorization required.
Responsibilities
- Provide comprehensive assessments and telephonic check-ins.
- Collaborate with care team to develop and execute care plans.
- Refer to community resources and maximize plan benefits.
- Assist with insurance benefits navigation with PRM.
- Deliver care coordination to improve health and reduce hospitalizations.
- Educate patients on advance care planning and end-of-life options.
- Document interventions timely for care teams.
- Provide guidance to Community Health Workers.
- Respond to crises with prioritization and multitasking.
- Other duties as assigned.
