Oak Street Health is hiring a Medical Social Worker to support psychosocial needs of high-risk patients. You will perform assessments, advocacy, and care coordination in collaboration with interdisciplinary care teams to improve health outcomes and avoid hospitalizations.

The role requires an MSW (CSWE-accredited) and licensure in the state, with multilingual capabilities and EMR proficiency. 40 hours weekly, full-time, in the New Haven/Connecticut area, with a comprehensive benefits package.

Qualifications

• MSW degree required from CSWE-accredited school.
• Minimally licensed in the state where Oak Street Health Center is located.
• Fluency in Spanish, Polish, Russian, or other languages spoken by the communities we serve.
• Proficient computer skills (Microsoft, GSuite, EMR).
• US work authorization required.

Responsibilities

• Provide comprehensive assessments and telephonic check-ins, plus in-person visits for high-need patients.
• Collaborate with patients and support persons to set goals and drive care plan execution.
• Refer to community resources to address healthcare and psychosocial needs.
• Navigate insurance benefits with the PRM to optimize plan benefits.
• Deliver longitudinal and episodic care coordination to empower patient self-management.
